The ECG app is not available in every country. Apple maintains a list of supported regions that changes with watchOS updates. Before assuming a hardware problem, confirm your Apple Watch model is supported, your region is on the list, and your age meets the minimum threshold. Why is the ECG app not available on my Apple Watch?

The ECG app requires a supported Apple Watch model (Series 4 or later, excluding SE), a supported country or region, and a user age of at least 22 in most regions. If your device, region, and age all qualify but the app still does not appear, check that your paired iPhone region is consistent and that the Health app has completed its initial setup.

Which Apple Watch models support ECG?

Apple introduced the ECG app with Apple Watch Series 4. All subsequent numbered Series models (Series 5 through Series 9, and the most recent Series) support it, as do both Apple Watch Ultra generations. The Apple Watch SE and Apple Watch SE 2 do not include the electrical heart sensor needed for ECG.

If you own a supported model and the app is still missing, the issue is almost certainly regional — not your hardware. One quick check: does a friend in the same country with the same watch model also lack the app? If yes, it is a region gate. If no, it is worth a deeper dive.
